Emergency Medical Associates (EMA), an Envision Physician Services provider, is recognized as a regional leader in the fields of emergency, urgent care and acute care medicine. We staff some of the most respected hospitals and health systems in the tri-state area. Our two wholly-owned EMedical Urgent Care centers have been serving the needs of the local community for more than 30 years.
Our physicians and advanced practice providers are fully supported on both the local/regional level (e.g. scribes, leadership academies, clinical labs, annual emergency and acute care medicine conference) and on the national level through Envision Physician Services’ robust clinical and operational support. This includes membership and participation in local, regional and national medical associations and societies.

John C. Bertolini, M.D., FACEP
Executive Vice President, Northeast Operating Unit
Dr. Bertolini is responsible for overall quality and for financial and operational performance of the Northeast Operating Unit (NEOU). Providers in the NEOU care for almost two million patients annually. Dr. Bertolini has more than 25 years of experience in medical group leadership.
Dr. Bertolini joined EMA in 1996 as the E.D. Director at Union Hospital. He served on the board of directors and held various executive positions in EMA prior to being named CEO in 2015.
Dr. Bertolini earned a Bachelor of Science in Biochemistry from Trinity College in Hartford, Connecticut. He graduated cum laude from Georgetown University School of Medicine in Washington, D.C. He then completed his internship in general surgery at Hartford Hospital and his residency in emergency medicine at Georgetown/George Washington University’s combined program in Washington, D.C. Dr. Bertolini is a member of the New Jersey chapter of the American College of Emergency Physicians and a diplomat of the American Board of Emergency Medicine. He is a Fellow of the American College of Emergency Physicians and a Fellow of the Advisory Board.
Fareed N. Fareed, M.D., MBA, FACEP
Senior Vice President, Northeast Operating Unit
Dr. Fareed has financial and operational responsibility for all of the New York sites and seven sites in New Jersey.
Dr. Fareed joined EMA in 2011 as Associate Director of HealthAlliance Hospital (HAHV) in Kingston, New York. He was E.D. Director at HAHV for five years and Regional Medical Director in New York for one year prior to becoming Senior Vice President in August 2016. He was an attending physician at NewYork-Presbyterian Columbia and core faculty in their EM residency program for six years prior to joining EMA.
Dr. Fareed graduated magna cum laude with a Bachelor of Science in Biochemistry from Harvard College. He earned his medical degree from the Columbia University College of Physicians and Surgeons. He then completed four years of EM residency at NYU/Bellevue Hospital Center. He received the MBA Academic Excellence Award from the Marist Graduate School of Management.
Douglas G. Smith, M.D., FACEP
Senior Vice President, Northeast Operating Unit
Dr. Smith has financial and operational responsibility for several EMA sites in the NEOU.
Dr. Smith has more than 20 years of clinical leadership, focusing on medical staff development, physician integration and healthcare informatics. He is responsible for the leadership and management of multiple hospitals in the mid-Atlantic and northeast United States with emergency medicine and hospitalist specialties.
Prior to joining Envision Physician Services, he served as the Chair of Emergency Medicine at Inova Fair Oaks Hospital, Vice President of a physician-led emergency medicine and hospitalist medicine management group and Senior Leader with the Inova Health System. Dr. Smith graduated from Virginia Tech and earned his medical degree from Eastern Virginia Medical School.
Dr. Smith recently retired from the United States Army Reserves, where he served for 34 years as an emergency physician, holding the rank of colonel at retirement.
